Rochester Outpatient
Our Rochester Outpatient provides an array of services necessary to create access and provide the appropriate treatment for those with substance use disorders. Our clinical professionals assist individuals with meeting their treatment and recovery goals. Coordination of care is done with other community supports as needed. Treatment goals are pursued and met through an individualized plan. Participants will meet regularly with their counselor utilizing evidenced based practices. Services are offered Monday through Friday.

This program serves all patients regardless of inability to pay. Discounts for essential services are offered based on family size and income. For more information, ask at the front desk.
Who we serve:
- Adults with substance use disorders
- Those with long-term chronic substance use disorders
- Under-served individuals and families
- Low-income individuals and families
- Individuals who are insured, uninsured or on Medicaid
- Individuals with complex health profiles
Services we offer include:
- Outpatient substance use services
- Low Threshold Buprenorphine Program
- Screening, assessment and diagnosis, including risk management
- Health screening and collaborative care/referrals to primary care
- Peer support, counseling services and family support services
- Connections with other providers and systems including criminal justice, education, foster care, child welfare, primary care facilities and hospital
Program Hours
Monday – Friday 8:00am – 4:30pm
